Entertain work permit from the industry did not come in 3 days?
I sent in the form of a pre-addressed also stamped around 2 weeks. I have yet to receive the work permit. Any ideas on what to do?

Assuming you live in North America. The only time you need a work permit for the industry Entertainment is whether you auditioned for and were booked in the use of the Union.
